COVID-19 Prospective Cohort Study:

We are sorry to hear you are having trouble and will happily assist you as soon as we can. Please click here to find the contact information for your site .
-
The goal of this study is to understand the impact of short and long-term outcomes of COVID-19 and its complications in children. To do this we need to collect the same data at different time points. Your responses are helpful in making sure the data represents your child and any changes that may or may not occur over the 12-month period of the study.
-
It is natural that not all questions will apply to your child or family. This can be especially true in scenarios where a child functions at an age/grade level that is different than their age. In these situations, please answer the questions to the best of your ability and skip only the questions that do not apply to you and your child.
-
We greatly appreciate your time and contribution to helping us comprehensively answer important child health questions, starting with COVID-19. As a token of our appreciation, for every timepoint completed, your name will be entered into a draw to win a $200 gift card of your choice. This means you will have up to five opportunities to win!

Administrative health data is information on health and health care visits. For example, when a child or adult visits the hospital as a patient, administrative health data is generated to document what the visit was for. This data helps to inform and enhance decision making and patient outcomes. When used for research, the data that is collected contains no personal identifiers.
